Who We Are
Pathways to Care (PTC) is a not for profit, licensed Assisted Living Facility, which provides medical respite to people experiencing homelessness. Opening in 2003, PTC serves as a post hospital resource for patients without a safe place to heal from injury, illness, or another medical setback. Our 34-bed program is an excellent discharge solution for health care providers. We provide services to approximately 200 residents annually.
We collaborate with and accept referrals from multiple hospital systems throughout Central Florida, Veteran’s Administration, Housing Opportunities for Persons with AIDS network (HOPWA), faith-based organizations, local governments, social service agencies and other medical professionals. In 2015, we expanded our services to include a transitional housing. program that promotes independence called Pathways Step2.
